Initial Comments:
An off-site follow-up survey conducted on November 7, 2022, found that The Belford, LLC, had corrected the following deficiency cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A. The deficiency was cited as the result of a complaint investigation survey completed March 17, 2022 and a follow up survey completed May 21, 2022.
